5 plus years of field nurse case management experience with a home healthcare agency.
Good driving record and a reliable vehicle.
Proficient in Oasis.
Excellent care planning skills.
Make a professional appearance.
RN required - CNS desired.
The RN / Clinical Nurse Case Managers responsibilities are:
Comprehensive clinical assesment.
Admission eligability determination.
Interdisciplanary care coordination.
Development and deligation of plan of care.
Periodic evaluation of plan of care.
