Growing Home Health Care Company in Galveston, Texas is seeking a Patient Care Coordinator who will coordinate all services for a defined group of patients so that the stated goals and objectives of the patient, physician and agency can be achieved.

Responsibilities:    
The Patient Care Coordinator must have the following skills and abilities:
  • Registered Nurse currently licensed in the State.
  • At least one (1) year of nursing experience, preferable six (6) months supervisory experience
  • One year of home health experience preferred
  • Demonstrates organizational and leadership ability.
  • Maintains current CPR certification.
  • Ability to transfer patients into and out of beds, wheelchairs and other devices.
  • Ability to transport self to and into patients' homes to perform duties as outlined in the care plan.
  • Ability to write notes and make records that can be entered into the medical record.
  • Ability to use the devices and techniques required by good nursing practice and the plan of care.
  • Ability to read the hand written and machine printed word.
  • Ability to learn new techniques and equipment to improve patient care delivery
  • Ability to tolerate stress and a fast paced environment
  • Good verbal and communication skills and ability to maintain a positive attitude
  • Ability to work over time as needed.

    Primary Duties and Responsibilities of the Patient Care Coordinator include:
  • Attends all called patient care coordination meetings.
  • Takes referrals and evaluates for agency eligibility.
  • Accepts responsibility for the overall care of each assigned patient.
  • Directs and leads a group of agency staff and contract staff to ensure the overall care of each assigned patient.
  • Assigns qualified agency staff to fulfill the physician’s orders and ensures that visit notes are received within the time prescribed.
  • Ensures that contracted staff fulfill the physician’s orders and that visit notes are received within the time prescribed.
  • Reviews all assessments, orders and notes for each patient assigned and ensures that all orders are sent to the physician in a timely manner.
  • Ensures that the plan of care is followed.
  • Writes discharge summaries and ensures they are sent to the patient’s physician.
  • Responsible for the staff orientation for all members of her/his team.
  • Conducts case conferences with clinical staff on a regular basis.
  • Ensures the clinical charts are complete and correct in that they meet the standards of practice and Medicare guidelines.
  • Writes the staff schedules and determines the appropriate scheduling of days off for team members in conjunction with the staffing coordinator
  • Ensures that the physician is notified appropriately for a significant change in patient status.
  • Required to report immediately employees for infraction to the Supervising Nurse and Administrator
  • Performs most supervisory visits and enough assessment visits to educate herself/himself as to the general status of each patient assigned.
  • Assures a reassessment of a client’s needs is performed by the appropriate health care professional in a timely manner on admission, when there is a significant health status change in the client condition at the physician’s request; after hospitalization; or upon discharge, if possible
  • Enforces dress code and other policies and procedures among nursing staff.
  • Produces the Home Health Aide assignment care plan and ensures that aide supervisory visits are performed at least every fourteen (14) days.
  • Rotates the RN staff for night, weekend and holiday after-hours calls.
  • Makes nursing care visits as required to meet the patient needs.
